ADHD & Adult Autism Screening - Before you consult with a GP to discuss a possible ADHD or Autism diagnosis or referral to a secondary care service, patients should download and complete the relevant forms below and return to practice via email to ggc.gp49144@nhs.scot or at our reception desk before your appointment.
Blood tests ordered by one of our GPs or nurses can be taken here (for elderly or infirm patients) or at the Central Phlebotomy Hub.
Cervical Smear Tests are usually performed by the practice nurses. The ideal time is midway between periods. We operate a recall system and contact women when their test is due. We recommend that women have a test at least every 3 years between the ages of 25 and 49 and then every 5 years between the ages of 50 and 64
Chronic Disease Management - The practice will monitor patients either 6 monthly or yearly depending on the number or severity of your chronic disease. If you suffer from any of the following conditions listed below, you will be invited for review. More information of each condition can be found by clicking the condition below:
Patients will receive an invite to book a blood test via your local phlebotomy hub and you should then arrange a Nurse review appointment in practice at least 2 weeks after your blood test appointment. Our Practice Nurse will review your blood test results and will take your blood pressure, height, weight and will discuss general lifestyle advice depending on your condition. We encourage patients to attend when you receive your invite.

Contraceptive Services, Family Planning & Female Health - Our family planning nurse is happy to give advice on contraception and sexual health. Intrauterine contraception devices (IUCD, Coils, Mirena) and the contraceptive implant can be fitted by appointment with one of the doctors. Emergency contraception is available at your local pharmacy. If you have booked a coil or implant appointment, the relevant consent form below should be completed prior to your appointment:

We kindly ask that patients complete the following forms if planning to consult a GP with regards to a new diagnosis or request for HRT
Maternity services are shared between your GP and the community midwife. You should contact the maternity services upon finding out that you are pregnant.
Mental Health Services - A range of services is provided at the practice and elsewhere by the Community Mental Health Team. If you think you might benefit from these services, please discuss the matter with your doctor. They may suggest sources of help to which you can refer yourself or they may refer you to the CMH Team. After an initial assessment you may be referred on to a counsellor or other professional.
Minor Operations and Dermatology Services are available at Nithsdale Merryvale Group Practice in accordance with criteria laid down by the Primary Care Trust. Please contact our reception or ask your Doctor for details.
Services for Young People - The Practice Nurse is available to see young people by appointment only. She can discuss anything that might be bothering you, e.g. skin problems, medical problems, contraception, sexual health, etc. She will decide whether you need to see a Doctor and can arrange this for you. Sometimes it can be difficult for young people to involve their parents in their concerns and it is our policy that everyone (regardless of age) has a right to confidential advice.
Sickness Certification - You are responsible for completing a Self Certificate for the first 7 days of any illness (Sundays are not counted), using form SC2, available from your employer, online at gov.uk. Your employer is legally bound to accept a Self Certificate and you do not need to see a doctor for this. After 7 days of sickness, Certificates are available from your doctor. Any other certificate requests will involve a charge.
